When do I need Palliative Care?

Do you or a loved one…

  • Have a serious illness or chronic condition that requires medical treatment?
  • Feel you would benefit from monitoring and assistance because of a recent hospitalization or illness due to diabetes or other medical conditions?
  • Need additional help with pain and symptom management or therapies to help return to prior living arrangement?
  • Feel uncomfortable with living at home alone?
  • Require transportation to medical or therapy appointments?

Examples of those who may benefit from Palliative Care at Emerald Gardens:

Bill had hip replacement surgery. He is weak from surgery, and has experienced dizzy spells.  He may require changes to his medications based on his ability to tolerate the side-effects.

Mary has been hospitalized for complications from Chronic Obstructive Pulmonary Disease. When she goes home, she forgets to take her medications and use her oxygen consistently.  She has fallen several times.

Cole is receiving chemotherapy.  After his treatments he feels weak for several days. During these times, he has no appetite and finds it difficult to eat well.
